AFFORDABLE CONNECTIVITY PROGRAM (ACP) DISCLOSURES AND CONSENT

The LTE AFFORDABLE CONNECTIVITY PROGRAM (LTE ACP) is a program created and managed by LTE WIRELESS to help low-income households with a discount on Mobile Broadband service and connected devices.

LTE ACP allows every member of a household to set up service, not just one.

Every member of your household can now have Talk, Text and Data services for as low as a $20 co-pay monthly

Everyone can also receive a Free Tablet and or a Free Smartphone with a Onetime co-pay of just $99 per device

The Program does not require you to accept a discounted device to be enrolled in LTE ACP, however, an eligble device is required to obtain broadband service from LTE Wireless. In case you accept a discounted device from LTE Wireless, the LTE ACP requires you to make a Onetime co-pay of no more than $99.

All Tablets from LTE WIRELESS works with our SIM cards

Most Phones from LTE WIRELESS have 2 physical SIM card slots. You may verify by emailing your question to ACP@LTEWIRELESS.COM

The LTE ACP provides Talk/Text and data services to be used on any unlocked Smartphone or Tablet We recommend you purchase devices from LTE WIRELESS.

ALL Co-Pay MUST be received and applied to your account before the 1st of the new month or your service will be interrupted.

If your Service is interrupted porting out to another carrier would NOT be allowed

If your service is interrupted for more than 30 days it will be permanently Disconnected

Any Phone number that has been disconnected CANNOT be reactivated.

There are NO refunds of Co-Payment if you cancel service or port out to another carrier.

There are NO refund of co-payment for Devices

You will investigate all devices and fully understand before making a co-pay or purchasing
